When compared to SICS-trab and phaco-trab, ECCE-trab has got so many disadvantages like more astigmatism, longer postoperative period, delayed visual rehabilitation and less IOP control. The key steps are making a good scleral flap, adequate capsulotomy, nucleus expression, cortex wash, IOL implantation, trabeculectomy and suturing. Since it involves larger area of conjunctival dissection, adequate care should be taken to have a water-tight conjunctival suturing.
