We describe a challenging scenario where an overhanging cystic bleb occluded the visual axis together with mature cataract. We performed simultaneous sequential horizontal bleb transection, phacoemulsification and intraocular lens implantation. This appeared safe and effective, with resolution of dysthesia, no bleb leaks, cleared visual axis and low-teens pressure at 4 months.
